Tourist Requirement Checklist

• National passport with validity of at least 3 months after the end of the planned journey
• Completed and signed application form with 1 recent color photograph (5x3cm) with white background glued on the application form ( Form Link ).
• Copy of the identity page of your passport and copy of the most recent Schengen visa with stamps
• Full detailed itinerary (day to day itinerary)
• Hotel reservation for the total duration of the stay in Belgium and in other countries in Schengen
• Letter of employer, stating your income and your vacation days allowed or – Copy of inscription in the commerce register or as a partner in a company or – Certificate from school stating your vacation days allowed (in case student)
• Proof of sufficient personal financial means like :
  • (a)Updated passbook recent 3month bank statement seal and signed.
  • (b)Income tax/Form16 papers of last 3 years.
  • (c)Salary slips last 3months.
  • (d)Self-employed – Incorporation or GST
• In case of a minor who has not reached the age of 18 years AND is travelling with one of the parents only or alone: a notary affidavit mentioning the consent of the parents must be submitted. This document needs to be apostilled.
• Copy of travel insurance valid in Belgium (and other Schengen countries), coverage minimum 30.000 Euro or equivalent amount in Rupees.
• Flight ticket reservation
• Old passports original need to be submitted.

Family or Private Visit Requirement Checklist

• National passport with validity of at least 3 months after the end of the planned journey
• Completed and signed application form with 1 recent color photograph (5x3cm) with white background glued on the application form ( Form Link ).
• Copy of the identity page of your passport and copy of the most recent Schengen visa with stamps
• Invitation of the person in Belgium stating the purpose of stay, address, telephone number, e-mail address and the period of stay of the guest(s).
• Original of a guarantee letter (according to article 3bis of law of 15/12/1980), legalized by the municipality where he/she is residing (+ 1 copy).
• Copy of the id card or residence permit of the guarantor, mentioning the right to reside in Belgium for an unlimited period.
• Proof of recent income of the guarantor: (3 last salary slip + child benefits) or last incometax assessment paper from the tax department in Belgium

• Confirmation letter of the company in India explaining the purpose and duration of the journey and the guarantee to cover all costs that may arise during the stay in Belgium
• Proof of family relationship (in case of family visit)
• Proof of sufficient personal financial means like :
